A 10-year follow-up study of a 6-month advance care planning intervention showed significant improvement in patients’ experiences and hospice utilization, with a reduction in end-of-life acute care use. Investigators also observed a sustainable long-term impact on emergency department use and hospitalizations, as well as a significant improvement in overall survival.
